Xbox 360 keeps saying disc is scratched or dirty. Whats wrong?

My xbox 360 keeps saying the disc is scratched or dirty. It only occurs when i put in game recquires alot of loading so oblivion and dragon age 1 and 2 dont work. I had similar problems or couldnt get certain games to load when other would. Could be hardware or software problem with the box itself. there are tons of people who have had similar problems. I think your disk might be scratched.
